Takayasu arteritis (ta) is a rare large-vessel vasculitis that may present with neurological complications. we report the case of a 22-year-old male developed recurrent transient vision loss, vertigo, and left hemiparesis. examination revealed absent upper limb pulses and carotid bruits. imaging showed carotid and subclavian artery occlusions with chronic and subacute brain infarcts, consistent with ta. he improved with corticosteroids and azathioprine. this case emphasizes the need to suspect ta in young stroke patients with absent pulses.
